You must fill out a form and attach supporting documentation to request a replacement. Each item must also be the original, a certified copy, or a notarized copy unless otherwise specified.

U.S. Passport Renewal Application Form DS-82.

Fill out DS-82 once you’ve met the criteria for renewal. When filling out the form, adhere to the directions. Any incorrect information or incomplete forms can cause a delay in the procedure.

Any applicant whose passport has been lost, destroyed, or stolen should fill out Form DS-11 rather than DS-82. Additionally, fill out Form DS-11 rather than DS-82 if your passport is in good shape but has a longer validity period than five years.

* If you have received a Social Security number, you must supply it.

* If you don’t have a Social Security number, you need to submit a statement that says, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration.” The statement needs to be signed and dated.

* Using Form, you can request a passport book, a passport card, or both papers.

* By marking the ‘big book’ box at the top of the DS-11, you can request a 52-page passport book at no extra charge.

• Proof of U.S Citizenship.

Your proof must be a tangible, certified original or certified copy. Any document with the official issuing authority’s seal or stamp is considered a certified copy. One of the following documents must be submitted:

* U.S. passport with full validity and no visible damage; expired passports are acceptable. When a document is/was * fully valid, it was good for 10 years for adults and 5 years for minors under 16.

Birth certificate issued in the US that satisfies the following criteria:

  • Issued by the birth state, county, or city.
  • Your full name, birth date, and place of birth are listed.
  • Gives the entire names of your parent(s).
  • Has the date been registered with the registry (within a year after birth)
  • Contains the seal of the issuing authority and the signature of the registrar.

* Birth Certification or Consular Report of Abroad Birth.
* Certificate of Naturalization.
* Citizenship Certificate. Bring a Photocopy of your of your proof of citizenship.

• Present your ID

One of the following identity documents (IDs) must be shown to the acceptance agent. You must be easily recognized by the ID.

* Unharmed U.S. passport book or passport card, whether current or expired.
* A driver’s license that is in-state and in good standing, or an improved license with a photo.
* Documentation of Naturalization.
* Document proving citizenship
* (City, County, State, or Federal) Employee I.D.
* U.S. military ID or dependent military ID
* Present (reliable) outsider’s passport
* Applicants with children who are U.S. citizens.
* A parent of a U.S. citizen kid who is applying for a visa frequently uses the U.S. Permanent Resident Card (Green Card).
* Trusted Traveler IDs, such as current FAST, SENTRI, Global Entry, and NEXUS cards.
* Native American tribal photo IDs and enhanced tribal cards.
